Join Our Team as a Senior Case Manager!

Our busy Professional Fiduciary and Case Management company is looking for a Senior Case Manager to join our dedicated team. We provide support to individuals who need health and/or financial management, including:

  • Seniors
  • People with special needs
  • Adults with serious mental illness (SMI)
  • Veterans
  • Children and adults recovering from significant medical or accident injuries
  • Others who need assistance managing their resources

What You’ll Do

As a Senior Case Manager, you’ll work closely with our principal fiduciary, office administration, finance staff, medical professionals, and community resources to make a real difference. Your responsibilities will include:

  • Coordinating care, placement, and government benefits
  • Conducting monthly home visits, hospital, and doctor visits
  • Reviewing medical evaluations and care plans
  • Guiding caregivers and agencies to ensure quality care
  • Managing fiduciary care plans and financial requests
  • Overseeing invoices and check requests

This role combines fieldwork and office work , so flexibility and strong organizational skills are key.

What We’re Looking For

  • Education: Bachelor’s or Master’s in Social Work, Public Administration, or a related field
  • Experience: Working with seniors, individuals with cognitive disabilities, mental illness, or serious injuries
  • Skills: Excellent communication, positive attitude, and a team-oriented mindset
  • Other: Three professional references required at the initial interview; job offer contingent on background check

After 12 months, you’ll take the licensing exam to become a Licensed Fiduciary —we’ll provide training and study materials.
